For Employees with Injuries under Current Treatment by Sequoia

If you are currently being treated at Sequoia for an existing work-related injury/illness, please contact one of the above providers for future follow-up care. It is recommended that your medical records concerning your case be forwarded to one of the new providers. When you schedule your follow-up appointment, inform the new clinic that you are a SLAC/Stanford employee. At your clinic appointment, complete the medical release form so Sequoia will release the medical information pertaining to your injury/illness.

Change in Medical Provider for Work-Related Injuries/Illnesses

In a letter dated March 12, 2007, Sequoia Hospital announced that their Occupational Health Program will be closing and will not be accepting new Workers’ Compensation injuries as of March 16, 2007. SLAC/Stanford University employees who experience work-related injuries/illnesses will no longer be treated by Sequoia Occupational Health in Palo Alto or Redwood City. The University had no control over this decision and we apologize for any inconvenience.
