Details for Non-SLAC Employees*

*Stanford or SLAC does not have Workers’ Compensation responsibilities for non-SLAC employees and will not process any bills or reports received from medical providers for non-SLAC employees.

The following information for handling a work-related accident, injury, illness, or exposure applies to:

  • subcontractors
  • temporary personnel from an agency
  • visitors
  • non-employee experimenters
  • collaborators
  • students from other institutions

Medical Treatment

If your injury or illness is a non-medical emergency, you may seek treatment from your physician or one of the following health care providers:

Adhesive bandages, which are available through SLAC Stores, may be used until you have follow-up medical care.

Note: If you seek medical assistance for your injury or illness, you must identify your employer to the medical provider, otherwise the medical provider will bill you directly.

Reporting Your Injury

Your Responsibilities

You should immediately report any work-related accident, injury, illness, or exposure to:

  • Your SLAC supervisor or University Technical Representative (UTR)
  • The SLAC Medical Department. If the SLAC Medical Department is closed, notify the Department by phone ext. 2281 or email: mggherman@slac.stanford.edu as soon as possible. Leave a message noting:
    • Your name
    • Nature of the injury
    • Time and date of the occurrence
    • Name of clinic or hospital where you received treatment

For SLAC to comply with federal reporting requirements, you are strongly encouraged to complete Part A of an Occupational Accident/Incident Report (SLAC SU-17) within 24 hours of the occurrence. Forms are available at the SLAC Medical Department.

Note: You should complete an SU-17 even if you do not seek medical treatment.

You are responsible for notifying your employer of any work-related injury or illness resulting from an accident, incident, or exposure. SLAC will not contact your employer. You may obtain a copy of your SU-17 to provide to your employer.

Responsibilities of your SLAC Supervisor or UTR

Upon learning of your work-related accident, injury, illness or exposure, your SLAC Supervisor or UTR may determine that you need medical care from a health care provider of your choice (such as the SLAC Medical Department, Stanford Hospital, Alliance Occupational Medicine or Workforce Medical or your physician). If so, your SLAC Supervisor or UTR should arrange for your transportation that does not compound the injury to a health care provider.

Your SLAC Supervisor or UTR is responsible for ensuring that the SLAC Medical Department is aware of your accident, injury, illness, or exposure. If the SLAC Medical Department is closed, your SLAC supervisor or UTR must provide the following information in a telephone message or email mggherman@SLAC.Stanford.edu:

  • Your name
  • The nature of your injury
  • Time and date of the occurrence

If you complete Part A of a SLAC SU-17, the SLAC Medical Department will send the SU-17 to your SLAC supervisor or UTR. Within five business days, your SLAC supervisor or UTR must complete and sign Part B (Supervisors/Investigators/UTR/POC Section) and return the form to the SLAC Workers’ Compensation Administrator at MS 11.

If  your supervisor or UTR does not receive an SU-17 within a few days of your injury, he or she must obtain an SU-17 from the SLAC Medical Department, complete Part B and return the form to the SLAC Workers’ Compensation Administrator at MS 11.

Note:  The supervisor or UTR may not provide a copy of his/her statement to the non-SLAC employee.

SLAC supervisors or UTRs must also notify the SLAC Contract Administrator, Purchasing Department, ext. 4692 or email: patten@SLAC.Stanford.edu of your work-related accident, injury, illness or exposure.
