Note: If you seek medical assistance for your injury or illness, you must identify your employer to the medical provider, otherwise the medical provider will bill you directly. For SLAC to comply with federal reporting requirements, you are strongly encouraged to complete an Occupational Injury/Illness Report - Employee First Report of Injury (SU-17A) within 24 hours of the occurrence. The SU-17A is available at the SLAC Medical Department and is also located on the Web at: http://www-group.slac.stanford.edu/esh/eshmanual/references/incidentsFormInvestigateSU17A.pdf. Note: You should complete an SU-17A even if you do not seek medical treatment. You are responsible for notifying your employer of any work-related injury or illness resulting from an accident, incident, or exposure. SLAC will not contact your employer. You may obtain a copy of your SU-17A to provide to your employer. Responsibilities of your SLAC Supervisor or UTRUpon learning of your work-related accident, injury, illness or exposure, your SLAC Supervisor or UTR may determine that you need medical care from a health care provider of your choice (such as the SLAC Medical Department, Stanford Hospital, Alliance Occupational Medicine or U.S. HealthWorks Medical Group or your physician). If so, your SLAC Supervisor or UTR should arrange for your transportation that does not compound the injury to a health care provider. Your SLAC Supervisor or UTR is responsible for ensuring that the SLAC Medical Department is aware of your accident, injury, illness, or exposure. If the SLAC Medical Department is closed, your SLAC supervisor or UTR must provide the following information in a telephone message or email mggherman@SLAC.Stanford.edu:
If you complete an Occupational Injury/Illness Report - Employee First Report of Injury (SU-17A), the SLAC Medical Department will send the SU-17A, and an Incident Investigation Form SU-17B, to your SLAC supervisor or UTR. Within three business days, your SLAC supervisor or UTR must complete and sign the SU-17B and return both forms to the SLAC Workers’ Compensation Administrator at MS 11. Note: The supervisor or UTR may not provide a copy of his/her statement to the non-SLAC employee. |
