Process

Affiliate

Individual who is not a user, but has some association with a SLAC activity for more than three (3) days and may need to have computer accounts in order to participate in a SharePoint site.  These appointments are reviewed annually.

Complete the on-line Affiliate/SU Employee Registration Form  and submit to HR Records.  The registration form and relevant training must be completed prior to being granted access to SLAC.  A System ID# will be assigned by HR Records.

SU Affiliation

SU staff, faculty, undergraduate, graduate student,  post-doc or SLAC medical staff who are affiliated with SLAC, need SLAC services and are not on SLAC’s payroll.  These appointments are for a maximum of one-year.

Complete the Affiliate/SU Employee Registration Form  and submit to HR Records.  The registration form and relevant training must be completed prior to being granted access to SLAC.  The System ID# is will be assigned by HR Records.

Users

Organization of scientists and engineers whose home institution has a relationship with SLAC and is doing research at SLAC. 

New Users must complete one of the following on-line forms: SLUO Users  or On-Site Users at SSRL & LCLS.

Visitors

Visitors are on-site three or less days and require an escort.

Visitors must complete the Site Entry Form  and the Vehicle Registration Form .   Security Office will handle Visitors on-boarding and exiting SLAC.

Contractors / Sub-Contractors

Contractors/subcontractors doing work at SLAC for a specific project or performing on-going services and/or maintenance.  Point of Contact (POC) must be an active SLAC employee and is responsible for assigning training requirements, advising HR Records when contractor/subcontractor appointment is ending and collecting any assigned SLAC property, including the SLAC ID badge.  These appointments are reviewed on an on-going basis.

Complete the on-line Contractor Registration Form.  The registration form and relevant training must be completed prior to being granted access to SLAC.  The System ID# is assigned by Procurement Operations or HR Record.

Temporary Agency Staff

Temporary Agency Staff are contracted through Kelly Services.

Hiring manager completes the SLAC/Kelly Services Order Request Form and is entered in PeopleSoft by the HR Coordinator.  Relevant training is coordinated through Kelly Services.

Department Associates

Retired or former SLAC employees providing consulting services on a voluntary basis and need services from SLAC during limited term or intermittent activities to support the sponsoring SLAC department.  The DA may retain email accounts only and must not be working in their prior position without compensation.  Additional approvals are required to maintain an office at SLAC and to ensure that safety and security training is up-to-date.  For more details  click here. 

Complete the Department Associates form and process required approvals.  HR Records enters information in PeopleSoft.
