Notification

You must notify the Foreign Visits and Assignments (FVA) Office if you plan to hire or invite any Sensitive Country foreign national who will be at SLAC for more than 30 days (assignment) in a 12-month period. There is no DOE requirement for FVA notification about Sensitive Country visitors, users, or guests who require access to SLAC for fewer than 30 days in a 12-month period.

Candidate should return completed/signed application and DOE FACTS Questionnaire to SLAC Employment on or before the interview date.  The FVA Coordinator will be contacted by SLAC Employment for further processing.
Users SLAC Users (Not SSRL or LCLS Users) No current DOE requirement

New users should complete the SLAC User Organization (SLUO) on-line form prior to arriving on site. Check in with SLUO Dept. with visa & passport documents. Contact SLUO (650-926-2363) for information.
Users SSRL and LCLS Users No current DOE requirement
(no FACTS entry)
User should complete the SSRL User Organization on-line form prior to arriving on site. Check in with visa & passport documents. Contact SSRL User Administration (650-926-3191) for information.
Visitors SLAC Site No current DOE requirement
(no FACTS entry)
Visitors should contact their SLAC Host on arrival and present visa and passport documents to the Office of Foreign Visits and Assignments (FVA) for processing into FACTS.  Contact FVA Office at (650) 926-5302 for information.
