underscores the importance of conducting detailed walk-downs to
ensure the isolation devices are correctly identified and
lockout/tagouts in the correct position.
2) After a lockout/tagout (LOTO) and an independent verification of the LOTO was performed on a high-capacity evaporator system in Building 9212, an NNSA Facility Representative was in the area reviewing the system and questioned the positioning of one of the 15 valve alignments. A critique was held and it was determined that the valve in question was in an incorrect position. There was no uncontrolled hazardous source in the system (the system had been drained) and an additional pump, under administrative control, remained as a barrier. Subsequently, the LOTO on the system was suspended and the valve lineup returned to an operational state.
Subcontract technicians who were replacing air lines on a range target system at the Savannah River Site did not utilize a proper lockout/tagout work package before performing electrical work on the target system's air compressor.
