Electrical/Fire Hazard from Cord contact with Footrest
To prevent a similar occurrence, inspect your work area frequently for unsafe conditions. Particular attention should be placed on power cords running underneath or behind furniture, maybe pushed tightly against cord causing damage, and an electrical short can occur. Follow these safety tips:

1) Keep power cords clear of mechanical devices (pinch points) and sharp objects that may cause cuts in the insulation.
2) If you need to reposition the power cord disconnect it from the power source.
3) If power cord shows signs of damage (frayed, cracked, or cut), do not attempt to make any repairs yourself, report it immediately to your manager/supervisor. Keep in mind most accidents usually occur as a result of faulty or defective equipment, unsafe installation, or misuse of equipment on the part of the employee.
 
Recently, an employee working at their work station heard a loud “pop” resonating from underneath their desk. A short time later, they heard another “pop” followed by some sparking and the smell of smoke. DOE LOTO Lesson Learned: Lockout/Tagout of Incorrect Isolation Point at Advanced Mixed Waste Treatment Project (AMWTP) Personnel not familiar with the rigors and formality of nuclear facility operations and strict compliance with LOTO requirements within the DOE environment may fail to adequately conform due to past work ethics acquired at previous jobs which encourages an independent "expert" approach to completing work activities.
