Building Managers, ES&H Coordinators, and University Technical
Representatives (UTRs).
Print the appropriate Focused Training Planning Form (see
below).
Review the appropriate program assessments with your employee.
Do this by locating
the program (see
below) the employee is in, and clicking the STA
Assessments link in that box.
Fill
out the appropriate planning form with your employee.
Enter the information into the
STA database, or take the completed form to your designated
STA entry person* to enter.
* If you don't know who your designated STA entry person is, ask your
ES&H Coordinator.
