All new
training programs need to be submitted for
review. This is to insure that the
training fits the needs of the lab and will
conform to our administrative requirements.
To submit a
course for review, please do the following:
- Fill out
the new
ES&H Course Proposal Form
- The form captures essential information such as:
- Course title
- Course description
- Prerequisites
- Who should attend
- Regulatory drivers
- How the course will be presented:
- Computer or web based
- Instructor led – who is the instructor? Outside vendor?
- The form once completed should be submitted to
Terry McMahon.
- If you need help filling out the form,
Terry or
Paul Bloom are available to assist you.
- Once the form is submitted, the course will be put in the queue
to be evaluated. These evaluations take place once a month at the
joint ESH - ESH Coordinator meetings. A review will be scheduled at
the earliest possible date. Further clarification and/or
specifics about the course may be requested at this meeting as a
condition of recommendation.
- Once the course is reviewed, a recommendation is submitted
to the ES&H
Training Group lead for approval. In certain
circumstances where the course impacts a large percentage of the
SLAC population, or has an otherwise high profile, approval by
the SLAC
Chief Safety Officer may be required.
- Once the course is approved, ESH Training will work with
the proposal submitter/SME to see that the
course is developed and entered into the course
catalog. ESH Training can also assist the training
submitter develop and execute communications to the training
target audience at the lab.
- If the course is developed by an internal (SLAC)
instructor, ES&H training should receive a lesson plan of
the course and a copy of any PowerPoint or other electronic
media used for the presentation.
- If you would like help in preparation of your course or
developing your lesson plan, contact ESH Training. We are
available to work with instructors on improving their
presentations or to simply provide constructive feedback.
- Every training course should have a lesson plan on file with
ESH Training.

- Below is a template that you can use:
Preparing your PowerPoint presentation and class materials:
An electronic copy of the latest version of your class materials should be
stored in the ESH Training folder on the V drive here - V:\ESH\ESH_Training\ESH
course materials\course XXX. The XXX is your course number. Every active ESH
Training course has it's own folder.
Samples of material that should be stored in this folder are:
- PowerPoint presentations
- Photos
- Video
- Lesson plans
- Other graphics and visual aids associated with the lesson
PowerPoint presentation Template - You should use the ESH
Training PowerPoint template for all your training presentations.
You can download a copy of the template

Template.
Emergency Information slide - In addition to the
PowerPoint template, we are providing a slide on Emergency
information that should be presented at the beginning of your class.
This slide goes in the beginning of your presentation.
Final Slide - We also have a slide which goes at the end
of your presentation which addresses our online class evaluation and
the process of receiving a training certificate of completion.

Modifying an existing course:
The process for modifying an existing course is similar to
proposing a new course but may or may not require the full review
process as described above. Review is determined
by the extent of the modification. This will be determined on a case
by case basis by the ESH Training Group Lead..
- Fill out the ES&H
Course Modification form
- This form is used to capture the changes that you are
proposing to an existing course.
- Submit the form to
Terry McMahon who will discuss the
modifications with you to determine if a committee review is
necessary.
- If no review is necessary, the modifications will be
implemented.
- If it is determined that a review is necessary, a
presentation at the ESH - ESH Coordinator meeting will be
scheduled.
- Approval from the ES&H Division head will be
required for major course modifications.
