Laser Operator and SLSO Restart Requirements
On Thursday, September 24 there was a laser
accident resulting in an injury to a laser operator. Following the
accident, a stop work order was issued for all Class 3B and Class 4
lasers in SLAC research laboratories. Information on the accident
and restart criteria for Class 3B and Class 4 lasers at SLAC is
provided at the internal Sharepoint site

The restart criteria require laser operators and System Laser Safety
Officers (SLSOs) receive additional training and be re-approved.
Important training information for these personnel to restart work
with 3B and 4 lasers follows.
QLO approval to restart laser work
following the 9/24 laser injury accident requires:
- Attendance at 9/25 Mandatory QLO
meeting; or if the QLO missed this meeting, s/he must review the 2
files listed below and meet with LSO to discuss (suggest this be
done after completion of the course in item 3).

- Complete new 253PRA course, which has
2 parts and is provided by the SLSO -- a common practical training
which takes about 45 minutes and site-specific training.
- Complete the new class “Laser Accident-Lessons Learned”.
Course Schedule: (all sessions are in the
Kavli Auditorium)
- Wed, 9/30 3:00 to 5:00 PM – for LCLS
Injector and NLCTA Injector laser personnel
- Thursday 10/1 10:30 AM to 12:30 PM - for NEH
Laser Hall and NEH Hutch 1 laser personnel.
- Thursday 10/1 3:00 to 5:00 PM - Other
laser personnel
-> additional courses will be scheduled as needed
SLSO approval requirements for laser
facility restart:
- ESH 130 Laser Supervisor Safety
course. This course is generally offered once per month, but a
special course time will be held:
- Friday 10/2 2:00-4:30 PM in the Kavli Auditorium

- The SLSO will meet individually with
the LSO to review readiness to return to safe operations with
adequate safety supervision for laser personnel.
- SLSOs must get re-approval
authorization from the LSO, program manager and ALD.
The ESH 130 course is required for SLSOs but
is also recommended to people who have supervisory roles for laser
personnel or facilities.
