Medical Care During Winter Shutdown
The SLAC Occupational Health Center is closed during the
winter
shutdown. As usual, if you have a serious injury, either you or
the reporting party should call 911 and then call SLAC Site
Security at x5555, then notify your supervisor.
If you have a work-related non-emergency injury, you may seek
treatment from the following healthcare provider:

Hours of operation for Alliance during this period are as
follows:
Monday - Friday, December 19-23: 8 AM – 5 PM
Monday, December 26: Closed

Monday, January 2: Closed
For any occurrences that require medical attention during
Alliance Occupational Medicine
closures, please go to Stanford Hospital Emergency Services
& Prompt Care. They are open 24 hours a day.

You must also notify the SLAC Occupational Health Center of
any occurrences x2281. Leave a message noting:
- Your name
- Nature of the injury
- Time and date of the occurrence
- Name of clinic or hospital where you received treatment
When the Occupational Health Center opens for business in
January, you will be sent an SU-17A to complete.
