Program
Lockout/Tagout (LOTO) is a set of practices and procedures to
safeguard personnel against the unexpected energization or startup of
machinery and equipment, or the release of hazardous energy
during service or maintenance activities.
Training
A minimum of two core training courses are required, #136 and
#157, to be qualified for lockout tagout work. A new annual refresher course, #157R,
is also required to insure that
frequent changes that electrical workers experience in their
assignments, procedures, etc. are updated uniformly site wide. Descriptions

Note: Course #157 includes #157R so that the latest changes in
requirements for electrical lockout tagout are presented to
persons taking #157.

To be additionally qualified to perform lockout tagout
zero-voltage verifications, #138 plus either #251 or #274, are
required.

A copy of the slides for course #157 is available for reference.
They are not a substitute for classroom training.
You must still take Course #157 to be qualified.

LOTO Tag Template
The lockout tagout program allows individual departments to issue
lockout tagout tags and lockout tagout locks to their authorized employees. Using these templates, information can be entered, tag printed out, cut out, folded over and laminated. Personnel pictures can be copied from the online phone directory or other source and sized in
MS WORD to fit the appropriate spot.

Annual Inspections
To complete Course #157R, the following form must be filled
out, signed, and sent in to ESH Training, MS 84.
