Authorizing Personnel (5.2.1)

• Requirements for designating system laser safety officers (see Laser Safety: System Laser Safety Officer Appointment Form [pdf])
• LSO interview for laser operators (see Laser Safety: LSO Laser Operator Interview Checklist [pdf])
• Additional requirements for qualifying non-employee laser operators (see Laser Safety: Safety Comes First Checklist for Non-employee Laser Operators [pdf])
• Additional requirements for qualifying student laser operators (see Laser Safety: Student Requirements [pdf]).

Authorizing Laser Laboratories for Operation (5.2.2)

• Requirements for reviewing and approving laser facility operation (see Laser Safety: Standard Operating Procedure Template [pdf] or [doc] and Laser Safety: Approval to Operate Form [pdf] or [doc])
• LSO checklist for laser lab inspection audit (see Laser Safety: LSO Laser Lab Visit Checklist [pdf] or [doc])

SOPs for Class 3b and Class 4 lasers will include responsibility assignments, laser descriptions, safety hazards, and required safety controls. More information on SOPs may be obtained from the LSO. Operation of all Class 3b and Class 4 lasers require signed, written approval of the LSO.
