JHAM FAQ Creating a JHAM Non-Routine JHAM    

All SLAC employees must use a JHAM process.  For users, subcontractors, and other non-employees, please see the Training Needs Assessment Procedure [pdf] to find out if you should do a JHAM process.

Format

There is no required format for the JHAM. An sample format is provided below. You can modify this format as long as the basic process is covered.  (Ask your supervisor what the standard for your department is.)

Instructions

Completing a JHAM is not complex. It requires focused thought about the steps needed to perform a job and the various things that can go wrong. Suggested steps are described below.

  1. List key steps required to perform a given task.

  2. To the right of each step, list potential hazards (things that can go wrong, how someone or something can get hurt or damaged). If you find that there is a large number of potential hazards for any step, consider breaking it into sub-steps and evaluating each individually.

  3. Identify the major controls for each of the identified hazards. Examples of controls can be to wear specific PPE, obtain a confined space permit, attend a specific training class, request ES&H review prior to initiating work, contact Waste Management to obtain appropriate hazardous waste containers, or follow a specific written procedure.

  4. List any associated training.  (optional)
    Note - this column was added to the template in 2006 by request of users.  It's purpose is to make it clearer what training was associated so they could also use it in their SLAC Training Assessments (STA).  It is an optional component, and the older templates without the column are still valid for JHAMs.

  5. As the JHAM is written, the employee(s) and supervisor should agree on the scope of the JHAM and discuss how the controls and recommended actions should be applied.

  6. The supervisor and the employee(s) sign the acknowledgement section of the JHAM.

  7. Copy of routine JHAMs should be retained by the employee and the supervisor, and should be updated at least annually. Copies of non-routine JHAMs should be retained by the employee(s) and the supervisor for the duration of the job. Non-routine JHAMs should be posted at the job site.

Examples

Here are some examples of how the JHAM can be used for people in different job situations. Each sample contains tasks, examples of hazards, and ways to mitigate those hazards. People using these samples should go over the examples with your supervisors and find ways to word them so it is clear to both of you.
