How to apply for a department account
SLAC employees who wish to become Card Custodians of an AMEX Department Account should obtain the forms listed below from the Travel Reimbursement Office. The forms are to be completed and returned for processing. (Note: These forms are not available online. They are only for internal use by Accounting.)

American Express Department Account Custodian Agreement and American Express Department Account Alternate Custodian Agreement. This form has two parts: Part One is to be completed by the party who will assume primary custodianship of the account. Part Two is to be completed by the party who will assume responsibility for the account during the absence of the primary Custodian.

Request for American Express Department Account and Designation of Custodian. The applicant's Department Head must sign this form.

Corporate Card Application / Department Custodian Card. The applicant must complete this form.
After The Above Forms Have Been Returned To The Travel Reimbursement Office:

The application will be forwarded directly to American Express on behalf of the applicant.

American Express will forward the AMEX card to the Travel Reimbursement Office in about two weeks.

The Travel Reimbursement Office will give a form with the credit card number to the applicant. The Account Custodian does not retain physical custody of the card.
